Effectiveness of Herpes Zoster Vaccine in Patients 60 Years and Older With End-stage Renal Disease

The shingles vaccine is not licensed for persons aged younger than 60 years. Once at the skin site, the virus may replicate more slowly than HSV. All authors: No reported conflicts. Subjects were followed for a mean duration of 3.13 years (range, 1 day to 4.90 years) and 95% of participants completed the trial. The messages should include facts supporting the importance of receiving vaccine and the impact of the zoster vaccination on the older adult population, citing high morbidity and a decline in the quality of life as the consequences of remaining unvaccinated. In another study, quality of life at 30 days was better with corticosteroid treatment (Whitley et al 1996); however, only adults without significant medical problems such as diabetes or high blood pressure were enrolled, resulting in a disproportionately healthy study population.

Separating the possible effects of the varicella and HZ vaccines on HZ epidemiology will be extremely challenging. The HZ Severity-of-Illness Score was defined as 0 for subjects who did not develop HZ during the study. PLoS Med 2013; 10: e1001420. We postulate that the lower HZ vaccination rate among AAs is partially the result of the lower self-reported prevalence of HZ and the lower rate of witnessing friends/family with HZ—all of which may influence perceived risk and therefore interest in vaccination. All symptoms resolved within 48 h. A sensitivity analysis was performed in which patients without known vaccine administration dates were censored at time of vaccine purchase.

Three hundred forty-nine participants (88.8%, 349/393) stated they would recommend the HZ vaccine to their patients. Furthermore, immunogenicity changed little with increasing age [14] and [15]. Carefully examine each ocular structure–from the cornea to the retina–in all pediatric patients who present with chickenpox. Higher coverage of vaccinations resulted in a greater increase in total costs each year. These problems may occur within minutes or a few hours after the vaccination, depending on the individual’s immune system. Although there has been concern about waning immunity, follow-up evaluations of children immunized during prelicensure clinical trials in the United States revealed protection for at least 11 years, and studies in Japan indicated protection for at least 20 years.19,,20 However, these studies were during a period when a substantial amount of wild-type VZV was present in the community, with many opportunities for boosting of immunity by subclinical infection in immunized persons.

After multivariable adjustment, vaccinated patients had significantly lower HZ risk compared to those not vaccinated in year 1 (RR 0.52, 95% CI 0.45-0.61), with waning protection over 5 years. So, vaccination would be a useful strategy. In early 2013, she developed sudden onset pain over her right hemithorax, which extended in a half-belt fashion from below the lower portion of the scapula, radiating forward along the right costal margin to the right upper quadrant of the abdomen. IFN-γ and IL-2 results for subjects ≥70 years old were significantly higher at baseline and after vaccination in the booster-dose group, compared with the first-dose group, indicating that a residual effect of ZV on VZV-specific CMI persisted for ≥10 years and was enhanced by the booster dose. It will help us if you say what assistive technology you use. Disseminated infection, defined as outbreaks with >20 vesicles outside the primary and immediately adjacent dermatomes, usually involves the skin and the visceral organs.

In multivariate analyses, physicians who strongly agreed that HZ and PHN cause significant burden were more likely to recommend the vaccine to patients 60–79 years of age (odds ratio [OR], 2.75 [95% confidence interval {CI}, 1.85–4.09]), whereas those who felt there was insufficient information about duration of protection (OR, 0.40 [CI, 0.24–0.67]), that the need to store HZ vaccine in a freezer was a definite barrier (OR, 0.31 [CI, 0.13–0.75]), or that their patients would not pay for the vaccine if it was not covered by insurance (OR, 0.57 [CI, 0.38–0.86]) were less likely to recommend it. The most common complication of HZ in elderly persons is postherpetic neuralgia (PHN), which frequently results in disordered sleep, chronic fatigue, anxiety and severe depression. Herpes zoster occurs in 1 million people each year in the U.S., and approximately one in every three people. Acyclovir, famcyclovir, and valacyclovir can all be used for treatment, although the latter two may be preferred because they require less frequent dosing.
